MADHOUSE features frequent, stylized violence and gore in a lo-fi pixel art style with chibi horror characters. Players engage in bloody combat against zombies and other playable characters in a four-player versus mode, with pixelated blood splatter effects, and encounter dark cinematic moments, such as the main character’s wife being killed in the opening cutscene. Despite the cute presentation, the game includes mature themes: strong language (e.g., occasional use of ‘fuck’), depictions of tobacco use (a teenage NPC smokes and requests cigarettes in an optional side quest), which references to underage smoking, a topic that may be sensitive for some players.
