Island of Hearts is a live-action romantic adventure intended for adult audiences. The game features frequent depictions of female characters in revealing clothing, such as bikinis, lingerie, and nightgowns. While there is no graphic nudity or visible genitalia, several scenes focus on sexual tension, flirting, and physical intimacy between consenting adults. This includes suggestive camera framing, dialogue with sexual connotations, and scenes where intimacy is strongly implied through context or fade-to-black transitions.
